Description:
This is a remote care coordination Medical Assistant role supporting value-based care (VBC) programs for cardiology patients.
Main responsibilities include:
Transitional care management (following up with patients after hospital discharge)
Patient outreach and follow-up calls
Care coordination and patient engagement
Intake calls (symptoms, vitals, surveys, social determinants of health)
Ensuring patients complete labs, imaging, and follow-up appointments
Escalating patient issues to nurses/providers when needed
Must-Have Requirements
Certified Medical Assistant (CMA)
AAMA or NCCT certification preferred
If certification is lapsed or expiring soon, the company will cover renewal
Experience in a medical office or healthcare environment
Strong phone communication skills
Comfortable working with KPIs and productivity metrics
Strong organization and multitasking skills
