Description:
- NPs will be assigned to Skilled Nursing Facility and become integrated with the facilities that the owner operates. Work out of those facilities daily (depends on the needs and membership on amount of time spent in each facility).
- The NPs collaborate with the PCPs and other RNs to create care plans focused on prevention, early detection and timely intervention.
- Typically overlook a membership of 65-85 patients. 8-5p schedule.
